Those three connections are presumaby between one of FM's servers and the MS one, or - even worse - between a single FM server (maybe always the same one) and the MS one.
I wonder if FM are certain that the count only applies to connections being made to send your traffic? I would have expected MS to be counting any/all connections from FM and thus the problem to occur if more than three of FM's customers' setups were simultaneously trying to send mail that way.
