I can get this to work by (1) having a matching 'Sending Identity' in Fastmail and (2) (under 'Advanced Preferences' at the bottom of the 'Preferences' screen) by deselecting the 'SMTP Identity' checkbox.
The text adjacent to the checkbox is

You could also change your default identity and leave the checkbox selected, but then all of your 3rd party client emails would have the same 'From' email address.
