Microsoft is testing Outlook.com Premium service. Up to five accounts with custom domain. Office 365 users will get it free.

Free with Office 365. I guess they might not release it as a separate product.

I have a grandfathered custom domain with 5 email addresses supported by Outlook.com & I have Office 365 Home.

I'd be keen to migrate to Outlook.com Premium.

Anybody know how I might be able to do this?

Can't do it now.

There's no word on when, or even if, Outlook.com Premium will become available to the general public or how much it will cost. Outlook.com already has an option for users to turn off banner ads on the site, along with technical support, for $19.95 a year.

Only partly true about being able to turnoff ads. Here in Ireland (despite being the location of Microsofts European HQ and a data centre ) we cannot avail of Ad-free Outlook.com . I find the banner ads intrusive and they eat up computing resources. Added to the fact that signing out of outlook.com drops you into MSN "news" I am thinking of cancelling my Microsoft account altogether.

I have received an invitation but was not able to try it out as I got an error: "We are experiencing issues with your request, try again later!" and then that my invitation was already used
However I managed to find out that it would be free for me for a first year (with 5 users) and then $3.99/month after that.

Is that $3.99/mo per user or $3.99/mo for all five?

Also, any word on whether the new backend will support the full Exchange protocol (and therefore work with Apple's desktop apps on OS X?)

I think it is 3.99$ for all five. It does support exchange and works with my Apple Mail.
They are also working on an option to move own domain to their servers. At the moment I was given an option to register a new domain with them at no charge.
