Yeah, I do tech support for a small ISP and we have stuff like this happen too. Users fall for scams, get malware etc... that results in their accounts being compromised and spamming, which can occasionally result in our outgoing server being blocked by some spam services.

Why is outbound email not subject to spam checks?

They are, but since these are accounts in good standing (ie paying customers that have been with us for a while) we do significantly less spam checking on them. Not none, but less than a new account.
