I have FM set up to forward "always" to another address just to keep a backup copy. When this happens, though, I get the email in the folder it gets filtered to, and I also get a copy in the inbox. ...

I can answer #3. Actually, I can only answer partially. I don't remember if you can actually use a +, but you can use a subdomain, i.e. amazon@username.fastmail.fm. This works for aliases as well....
